NOW OPEN: FinalForms registration for the 2026-27 School Year.  *When you log into your FinalForms account, click on the blue button and follow the prompts to register.  https://westernboone-in.finalforms.com/ FinalForms will rollover to the 2026-27 school year on MAY 24TH. You must register your student-athletes for the 2026-27 school year on FinalForms, manage their sports, upload a new athletic physical dated after April 1, 2026, and sign off on all required forms (new student athletic handbook, waivers, etc). To receive communications from coaches throughout the summer and for the 2026-27 school year through FinalForms, and to participate in athletics during the 2026-27 school year, this registration process must be completed. See detailed information about the 2026-27 FinalForms athletic registration process and athletic physicals below and attached. Athletic Physicals: All students (grades 5-12) participating in athletics in 2026-27 must have a new athletic physical completed and on file in FinalForms prior to participating in their first official practice/tryout of the 2026-27 school year. The new physical must be completed on or after April 1, 2026. Note: Last school years' 2025-26 Athletic Physical cover's athlete participation in Summer 2026 activities. The athletic office must have a copy of the Physical Examination page (pg. 3) with a doctor or practitioners’ signature at the bottom. This page must be uploaded and scanned to your student-athletes FinalForms account. • Scan or take pictures at any time and upload the Physical Examination page (pg. 3) to your student's FinalForms account yourself (this the PRIMARY METHOD of turn in). • If you need assistance with uploading it, you may call the Athletic Office during normal business hours on or after Monday, July 27, 2026, to receive help with uploading it. • DO NOT email them and DO NOT turn them into your coach. Note: The athletic office will be closed for the summer after May 29, 2026. Physicals that are uploaded after May 29, 2026, will NOT begin being processed until July 27, 2026. This means that your athlete may still show up as "Red' in FinalForms if you upload your athletic physical prior to July 27, 2026. The athletic office will process athletic physicals that have been uploaded over the summer and turn athletes "green" on and after July 27, 2026. FinalForms must be updated for the 2026-27 school year. Please follow the directions in the attached link.
